Peter, I think the regulators do what they can with the resources aloocated to them and within the political parameters allowed to them. However, the primary OHS duty and legislstive obligation has been squarely on employers since at least the 1980s. Employers are not passive participants and must carry a lot of responsibility for the lack of prgress on many workplace hazards.
